I agree to share my identity information along with my Aadhaar number with the State and Central Governments for the purpose of e-KYC or 'Yes'/'No' authentication with the Unique Identification Authority of India (UIDAI) for the Digital Public Infrastructure (DPI) being created for agriculture in the country by name AgriStack.

I also give consent to the State and Central Governments to seed my data into the Farmer Registry to be used for the implementation of this project, for delivering various current and upcoming services, and for providing current and upcoming welfare benefits of the State and Central Governments, including making payments through Direct Benefit Transfer.

Furthermore, I give my consent to use my data available in Pradhan Mantri Kisan Samman Nidhi, Pradhan Mantri Fasal Bima Yojana, State Direct Benefit Transfer Project, and other schemes of the Agriculture Department, as well as Record of Rights data of the Revenue Department, for the creation of required Farmer Registry databases to be used for the Digital Public Infrastructure.

I do hereby, on a voluntary basis, authorize the Board of Revenue Uttar Pradesh / National Informatics Centre (NIC) to share my Aadhaar data (excluding my biometric data), provided by me for State Farmer Registry under the Agri Stack Scheme of Government of India, with the Revenue Department Land Record Database.

हिंदी: मैं अपनी पहचान संबंधी जानकारी और अपने आधार संख्या को राज्य और केन्द्रीय सरकारों के साथ ई-केवाईसी या 'हां'/'नहीं' प्रमाणीकरण के उद्देश्य से भारत सरकार के (UIDAI) के साथ साझा करने के लिए सहमति व्यक्त करता हूँ, ताकि देश में कृषि के लिए 'एग्रीस्टैक' नामक डिजिटल सार्वजनिक संरचना (DPI) बनाई जा सके।

मैं, एतद् द्वारा स्वैच्छिक आधार पर, राजस्व परिषद उ०प्र०/राष्ट्रीय सूचना विज्ञान केंद्र (एन०आई०सी०) को भारत सरकार के एग्री स्टैक योजना के अन्तर्गत स्टेट फार्मर रजिस्ट्री हेतु उपलब्ध कराए गए मेरे आधार संबंधी डाटा (मेरे बायोमेट्रिक डेटा को छोड़कर) को राजस्व विभाग के डेटाबेस के साथ साझा करने के लिए अधिकृत करता/करती हूँ।
